We welcome all inquiries about our services.
The applicant and caregivers are invited to visit our Center and meet our staff. At this time, we will collect general information about the needs of the applicant, his/her daily routine, and activities she/he enjoys. We will also provide a more detailed explanation of our program.
A packet of forms and information will be provided and reviewed. Since Forster Woods Adult Day Center is monitored by Tri-County Office of Aging, our forms are in accordance with operating standards issued for Adult Day Services.
Following this initial assessment of the applicant a FREE VISIT day will be offered and scheduled. This provides an opportunity for the applicant to experience the safe environment and activities of the day. It also provides the staff an opportunity to determine the needs of the person.
Setting a weekly schedule is important for various reasons. Having a set routine not only provides some structure for the participant but also enables you, the caregiver, to plan your activities. In addition, the Center needs to determine the meals and staffing based on the number of clients attending. While we offer the options of either hourly, full days (up to 12 hours) or half days (up to four hours), we recommend that clients attend at least two days a week.
As a non-profit agency, Forster Woods is dedicated to keeping our rates affordable.
Participation in our program encourages our participants to maintain his/her independence, and in some cases, delays or prevents placement in a facility. In addition, caring for your loved one includes caring for you, the caregiver. If you need to work, are tired and need a break or are experiencing health issues, we can share the caregiving with you.
Fee Schedule
Many participants receive funding through state and local agencies that cover all or a portion of the costs. Forster Woods has contracts with Tri-County Office of Aging’s Medicaid Waiver program as well as Community Mental Health’s Community Living Support program. Some participants pay through long-term care insurance or are private pay. Grant Program is available for partial assistance.
